Colorado State Innovation Model (SIM)

SIM is a four-year, federally funded, health reform initiative that invested up to $65 million from the Centers for Medicare &, Medicaid Services in state infrastructure and practice transformation efforts that will improve patient health and prepare health providers for alternative payment models that reward the value (not volume) of care delivered.

The initiative, which ends in July 2019, has worked with 328 practice sites and four community mental health centers to integrate behavioral and physical health in primary care settings. The Colorado approach included investments in health information technology, payment reform and population health to ensure sustainable success with health reform efforts so that patients get the care they need when and where they need it:
